You acknowledge that this time and space is meant for authentic exploration of feelings and expression, collectively...
You are in charge of your choices and actions and know you are able to choose something different that what is being suggested or offered within the class if needed. You can quietly make a different choice or speak to the facilitator if needed.
You acknowledge that a level of conscientiousness to the group, its individual participants and space is a part of what it is to be in this shared space.
You commit to doing my best to balancing my needs of expression while attuning to the needs of the space and the group and holding these both at the same time.
RELEASE OF LIABILITY
This Informed Consent and Assumption of Risk and Release of Liability (“Release”) is made on the date specified via payment, between the party registering for J’ouvert Playhouse and Nicole Pemberton / The Goddess Moves and the Calgary Dance Collective.
INFORMED CONSENT. You acknowledges, certifies, and accepts the following:
- Physical Condition. That they are of adequate physical condition to participate in the guided class despite any current medical conditions they may possess.
- Assumption of Risk. That they assume the risk of physical injury from any guidance, instruction, or action conducted during or as a result of the class.
- Reporting Discomfort. That injury, discomfort, distress, or uncomfortable feelings will be brought to the attention of the facilitator when deemed of concern.
- Indemnification. That they will NOT hold the facilitator, Nicole Pemberton, co-producer, affiliates, agents, or any other entity or individual connected to them, either directly or indirectly, liable for any result from the .
- Responsibility. That they, the Participant, assume all responsibility for their participation in the Sessions.
By registering and submitting payment for this event you agree to assume risk and take full responsibility for your participation in any classes or events run by Nicole Pemberton of The Goddess Moves / Calgary Dance Collective.
MEDIA AGREEMENT
Calgary Dance Collective commit to using any content we capture during/before or after class tastefully and considerately.
Any content we capture is meant to serve and promote the spirit of the class, sharing this ethos and inviting others to participate in it.

You grant permission to Calgary Dance Collective_, hereinafter known as the “Facilitators” to use my image (photographs and/or video) for use in media publications including - social media postings, website content, newsletter and email blasts, printed materials.
(If you do not grant permission it is your responsibility to notify Nicole Pemberton via email nicole@thegoddessmoves.space or in person conversation before the event starts. Individual written/verbal Requests to not having images captured for promotional material will be honoured.)
You hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and You waive any right to royalties or other compensation arising from or related to the use of the image.
You are 18 years of age or older and I am competent to contract in my own name. You have read this release before submitting payment and fully understand the contents, meaning and impact of this release.
You understand that you are free to address any specific questions regarding this release verbally to Nicole or another member of the Calgary Dance Collective and then by submitting those questions in writing prior to attending Saturdays dance, and you agree that in failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this agreement*
The Goddess Moves/Calgary Dance Collective community is an inclusive safe space for all. Any harmful or disrespectful behavior that violates the safety of the facilitator/participants will result in you removed from the in-person experience immediately.
